Request Medical Records

If you’d like to access your medical records online, you can do so using My Health Rec, an easy to use online portal just for you. Access the Patient Portal.

If you’d like a copy of your medical records, please download the Patient Release of Information and Consent for Email Communication, then print it out, complete the required fields, sign and bring it to the Medical Records Department on the first floor of the hospital, or you can email to

When you arrive to pick up your records, please bring a government-issued photo ID. If someone else will be picking up the records on your behalf, that person will also need a photo ID along with an original signed authorization letter from you. Creating a copy of your records may incur a charge.
